Each academic year, all Monmouth College students registered for six or more semester hours are required to participate in the
Student Medical Insurance Plan unless an online waiver form is completed.
A summary of the Student Plan benefits
is available online at
www.gallagherkoster.com. For the 2009-10 school year, the
coverage is effective from August 10, 2009 through August 9, 2010.
The
2009-2010 annual premium is $508.
After reviewing the student insurance
coverage, if you believe you currently have adequate coverage and would
like to waiver the student plan, log onto
www.gallagherkoster.com,
click on 'College and University Students' and select Monmouth College
from the drop down menu. Click on 'Student Waive Forms' and select
the '2009-2010 Monmouth College Waiver Form.' Please have your current
health insurance ID card ready as you will need this information in
order to complete the waiver form.
The deadline to waive the student
insurance is August 1, 2009. A new
waiver form is required each academic year. Any student who has
not completed the waiver form will automatically be enrolled in the
student insurance plan and their student account charged the $508
premium. This is an online process only.
Paper submissions, letters, phone calls or faxes will not be accepted.
